As a medical social worker for AHN population health, and I provide support & healthcare navigation for patients and their families. I love my role because I am able to provide patients with resources, support for emotional & social needs, and assist with coordination of care alongside their medical providers.
The role of a medical social worker is vital for safe & seamless transitions of care, in which they connect patients and their families with necessary resources. I experienced the importance of this role early on in my own personal family life; which enriched my background of knowledge to help people through difficult times.

My primary role is to find much needed resources and assist my patients with application. The end result is to alleviate undue stress to their already chaotic situation. When and if counsel is needed, I provide a much needed ear and level of reason that many times allows patients or their families to see things clearly and avoid stress. In essence, I navigate with the patient and family a way to a brighter life.

The services I provide to patients connect them with resources that improve their holistic health. As a Master’s level licensed Social Worker I am trained to see the unseen. I assist Patients through assessment and evaluation to identify their needs and then bridge them to community entities that allow them to thrive in their preferred settings. My role makes a difference as it’s those one on one moments that patients feel heard and that they truly feel they have a support net and an advocate that genuinely cares and wants the best for them.
