As an RN Transition Coach, I ensure patients understand new health instructions received at discharge from a hospitalization. Many patients are discharged with new prescriptions and health habits. My role allows reinforcement and discussion optimizing stable/improved health statuses for my patients.

I am a health coach that works with high risk patients that have chronic and/or new health issues. I am a resource to them for education about their illnesses, their medication and help with any ancillary services they might need to better manage their health. I have found that my patients appreciate the calls and encouragement to make improvements in the managing of their health issues that can lead to a better quality of life for them. That in itself is the rewarding part of my job!

An RN Health Coach works with the patient to help them achieve an optimal level of perceived health and maintain wellness in light of such diagnoses as diabetes, Cardio-Vascular Disease, Heart Attack, etc. Wellness is not simply the absence of illness or dysfunctionality. It is the optimal level of perceived performance and resultant functionality encompassing all of life. People want answers as to how an illness or dysfunction will affect life. They need empowerment to attain achievable goals. An RN Health Coach is able to partner with the client to put together a strategic plan and include referrals that will provide ways to move at their own pace in accomplishing goals. Once a plan is established, the RN Health Coach assists to find solutions with any roadblocks that occur. Empowering someone with information, and helping them to recognize the most important lifestyle change that leads to an optimal level of perceived health and maintained wellness.

I help my clients and patients focus more on their optimal health and wellbeing by actively engaging them in conversation regarding their current health issues, in a supportive and caring way.  Educating patients, giving them the keys to make healthier life choices and decisions along life’s journey, is also a very important role in my job too.
